In 60s and later, a healthy balance of hormones is one of the crucial factors for maintaining muscle mass, bone strength, sexual health and independence for the activities of daily living .
It’s important to note that reduced testosterone does not necessarily constitute a medical condition. Many men continue to have healthy levels as they age and some men with low levels do not have symptoms. The clinical question is whether there is a consistent hormone deficiency contributing to meaningful changes in how you feel or function.
Normal Testosterone Levels By Age

The amount of testosterone is typically reported in units called nanograms per deciliter (ng/dL).
One frequently cited reference range for healthy, non-obese men between ages 19 and 39 is 264 to 916 ng/dL. This range came from a study that harmonized testosterone measurements across several large U.S. and European cohorts.
It is not a universal reference range for every adult man. Laboratories use different testing methods and may provide different normal ranges. Age, weight, illness, medications, and other factors can also affect the result.

The following testosterone levels by age in men are broad population estimates. They can help readers understand the general pattern of age-related decline, but they should not be used as stand-alone diagnostic cutoffs.
- Ages 20 to 29: approximately 300 to 1,050 ng/dL

Free Testosterone vs Total Testosterone
Total testosterone measures nearly all the testosterone circulating in your bloodstream.
Most testosterone is attached to proteins, primarily sex hormone-binding globulin, known as SHBG, and albumin. Only a small percentage circulates as free testosterone.
Free testosterone is the portion that is not bound to these proteins. It is often described as the testosterone that is most readily available for the body to use.

Free testosterone levels by age may fall within these broad educational ranges:
- Ages 20 to 29: approximately 9 to 30 ng/dL
- Ages 30 to 39: approximately 8 to 28 ng/dL
- Ages 40 to 49: approximately 5.3 to 26.3 ng/dL
- Ages 50 to 59: approximately 4.2 to 22.2 ng/dL
- Ages 60 to 69: approximately 3.7 to 18.9 ng/dL
- Ages 70 and older: approximately 2.2 to 14.7 ng/dL
Free testosterone testing is less standardized than total testosterone testing. Laboratories may use different methods and report results in ng/dL, pg/mL, or other units. Always compare your result with the reference range and unit shown on your own laboratory report.
SHBG can significantly affect how total testosterone should be interpreted.
When SHBG is high, total testosterone may appear normal even though the free portion is relatively low. When SHBG is low, total testosterone may appear low while free testosterone remains within range.
Age, obesity, thyroid conditions, liver disease, diabetes, and certain medications can affect SHBG. A provider may order total testosterone, free testosterone, and SHBG together when the initial result is borderline or does not match the symptoms.
What Counts as Low Testosterone?
The American Urological Association uses a total testosterone level below 300 ng/dL as a reasonable cutoff supporting a diagnosis of testosterone deficiency.
However, one low result is not enough.
A clinical diagnosis generally requires:
- Symptoms or signs associated with testosterone deficiency
- Two separate total testosterone measurements
- Both tests performed early in the morning
- Consistently low results
- Evaluation for possible underlying causes
The AUA also states that low testosterone should only be diagnosed when low laboratory values are combined with relevant symptoms or signs.
Symptoms may occur in men whose total testosterone is above 300 ng/dL, including men in the 400s. That does not automatically mean testosterone is the cause.
Fatigue, low libido, poor focus, reduced motivation, and difficulty recovering can also be related to sleep apnea, thyroid disease, anemia, depression, stress, obesity, medications, or excessive training.
Numbers matter, but they do not tell the full story. Symptoms, laboratory findings, testing conditions, and overall health should be evaluated together.
The Athletic vs Sedentary Context
The same level of testosterone can have drastically different effects on two men.
A 50-year-old individual who is sedentary (sedentary lifestyle) may have no symptoms at all if they have a level of 500 ng/dL. He might sleep well, have a good sex drive and have no worries about energy or day-to-day function.
At the same time, a 40-year-old healthy athlete with the same levels may complain of fatigue, decreased training ability, or poor recovery. And in that case, that is a warning sign.
But this does not imply that the man who is athletic should have a higher level of testosterone. Other factors to consider include overtraining, training volume, calorie intake, stress, sleep, thyroid function, iron level, and medications.
Being an athlete isn't enough to justify the diagnostic standard. When determining the meaning of the symptoms, the context is important as it will help the provider understand what might be contributing to the symptoms.

What About the 400 to 500 Range?
Many men receive a total testosterone result between 400 and 500 ng/dL and are told that everything is normal.
For many men, it is normal.
A result in this range is above the common AUA cutoff and does not, on its own, establish testosterone deficiency. Persistent symptoms still deserve attention, but the next step should be further evaluation rather than an automatic assumption that TRT is needed.
A provider may recommend:
- Repeating the test early in the morning
- Checking free testosterone and SHBG
- Evaluating sleep quality or possible sleep apnea
- Screening for thyroid problems or anemia
- Reviewing medications
- Looking at stress, nutrition, alcohol use, and training volume
- Checking for metabolic or cardiovascular risk factors
The Endocrine Society emphasizes that symptoms alone are not diagnostic. Reversible contributors, including obesity and medication use, should be considered before treatment.

What Is Considered High Testosterone?
There is no universal number that defines high testosterone for every man.
A result above the upper limit listed by your laboratory deserves review, particularly when it remains elevated on repeat testing or occurs during testosterone therapy.
Levels above 1,000 ng/dL may exceed the usual treatment target for many patients, although the significance depends on the laboratory method, age, timing, symptoms, and clinical context.
Excess testosterone exposure may contribute to:
- Acne or oily skin
- Increased hematocrit
- Reduced sperm production
- Testicular shrinkage
- Mood changes
- Higher estradiol levels
- Breast tenderness or gynecomastia in some men
- Increased blood pressure
Testosterone stimulates red blood cell production. When hematocrit rises too high, a provider may need to adjust treatment or investigate other contributing factors.
In 2025, the FDA required testosterone products to carry updated blood pressure warnings after ambulatory monitoring studies found class-wide increases in blood pressure. The same update removed boxed-warning language about increased cardiovascular outcomes after the TRAVERSE trial did not find an increased rate of major cardiovascular events.
The goal of medically supervised treatment is not to push testosterone as high as possible. It is to reach an appropriate physiologic level while monitoring safety and response.
Symptoms of Low Testosterone to Watch For
Possible symptoms of low testosterone include:
- Persistent fatigue that rest does not resolve
- Reduced libido
- Fewer spontaneous or morning erections
- Changes in erectile function
- Brain fog or difficulty concentrating
- Reduced motivation
- Irritability or low mood
- Loss of muscle mass or strength
- Increased body fat
- Reduced exercise endurance
- Poor recovery after training
- Low bone density
- Infertility
- Unexplained anemia
These symptoms can be significant, but they are not specific to testosterone deficiency.

How to Test Your Testosterone Levels
An early-morning blood test is the standard starting point.
Testosterone levels fluctuate throughout the day and are generally highest in the morning, especially in younger men. Testing should usually be performed between approximately 7 and 10 a.m.
A low result should be confirmed with a second morning test on a separate day. The Endocrine Society also recommends repeating a morning fasting total testosterone measurement before making a diagnosis.
A comprehensive hormone panel may include:
- Total testosterone
- Free testosterone
- SHBG
- Estradiol
- Luteinizing hormone, or LH
- Follicle-stimulating hormone, or FSH
- Prolactin when clinically appropriate
- Complete blood count
- Thyroid or metabolic testing based on symptoms
LH and FSH help a provider investigate whether low testosterone may be related to the testicles, pituitary gland, or hypothalamus.
Testing may be arranged through a primary care doctor, endocrinologist, urologist, or telehealth platform that coordinates laboratory work and provider consultations.

When Should You Consider Testosterone Replacement Therapy?
Testosterone replacement therapy is generally considered when two conditions are present:
- Testosterone levels are consistently low on appropriate testing.

The AUA supports using a total testosterone level below 300 ng/dL as part of that assessment. The Endocrine Society recommends treatment for men with symptoms and unequivocally, consistently low testosterone concentrations.

- Address possible sleep apnea
